Over the years that the insurance program has been running, many of you have asked us to look for a plan that covers our own personal health benefits.

We have been working with an insurance broker to negotiate such a program.

Please see the attached survey for various options that we have made available and their cost. The cost for each benefit is approximate. The insurance company will decide which benefits it will provide and the exact cost according to our response. To determine the level of interest in this program we would appreciate if you took a few moments to fill out our survey and return it to me by e-mail or mail.
